First, assuming you haven’t already lost your vaccination card, it’s a good idea to take a picture of it. Make sure the picture is clear and high quality so you can easily see what’s written on it. If you have a clear picture of your vaccination card, that’s usually all you need as proof of vaccination. If you have a clear picture of your card on your phone, you can usually leave your card at home where it’s less likely to get lost. Perhaps you’ve already lost your vaccine card. Don’t panic. You can get another one. You have three options.

  1. You can contact the place where you got vaccinated. They should be able to print out another vaccination card for you.

  3. Fill out the Immunization Record Request Form. You will be able to get a record of all the vaccines you have gotten, not just the covid vaccine.
