Here’s What You Should Do If You Lose Your Vaccination Card
You did it. You rolled up your sleeve and got vaccinated for Covid-19. In fact, you’re fully vaccinated, and you have the vaccination card to prove it. That’s great, because you might need that card for proof of vaccination if you’re traveling, attending a Broadway play, or if your workplace requires vaccination.
Your Covid vaccination card provides very important information. It’s important to keep it somewhere safe where you won’t lose it. Your wallet might be a good place, for example.
What if your wallet is stollen? What if you misplace your vaccination card? What if your child rips it up or flushes it down the toilet? Seriously. It could happen, and it’s best to know what to do if this important card comes up missing.
First, assuming you haven’t already lost your vaccination card, it’s a good idea to take a picture of it. Make sure the picture is clear and high quality so you can easily see what’s written on it. If you have a clear picture of your vaccination card, that’s usually all you need as proof of vaccination. If you have a clear picture of your card on your phone, you can usually leave your card at home where it’s less likely to get lost. Perhaps you’ve already lost your vaccine card. Don’t panic. You can get another one. You have three options.
- You can contact the place where you got vaccinated. They should be able to print out another vaccination card for you.
- Did you enroll in v-safe when you got vaccinated? If so, your vaccine information is stored there. It’s not too late to enroll.
- Fill out the Immunization Record Request Form. You will be able to get a record of all the vaccines you have gotten, not just the covid vaccine. If you need proof of covid vaccination right away, this is not the best method. It can take up to six weeks, and it needs to be mailed and notarized.
